As Emergency Department Intermediate Care Technician
for the VA Caribbean Healthcare System in San Juan, Puerto Rico, the incumbent performs ongoing patient care duties in the Emergency Department. The incumbent provides assessment and management of ambulatory patients seen in the Emergency Department to include obtaining patient health history, chief complaints, medication history, and identification of health-related conditions that may impact the patient's health status. Other duties include, but are not limited to:
Manage would dressing changes, suture removal, and repair of simple lacerations.
Set up sterile field with all required items for each procedure such as air-powered equipment, fiber optic equipment, laparoscopic equipment, microscope, lasers, and other procedure room supplies and equipment.
Foley catheters insertion, splint, binder and bandage applications, and performance of tracheotomy care.
Collect specimen and prepare for transfer to lab for processing of blood, tissue, urine, stool, and sputum is expected to be a routine practice.
Document actions, assessments, diagnostics, and procedures in the medical record.
Respond to medical emergencies on the facility grounds.
Transport monitored and unmonitored patients to diagnostic procedures and/or inpatient units.
Coordinate with ancillary services for safe and efficient movement of patients to diagnostic and therapeutic procedures/studies.
Identifies the nature and extent of a patient's condition or seriousness of the injuries or illness.
Makes assessment of the patient's emergency care requirements.
Assists physicians with airway management under emergency situations, provides cardiac resuscitation, assisting physicians with procedures such as thoracotomies, chest tubes, invasive line placements, etc. as appropriate.
Rotating shifts Monday through Sunday, weekends and holidays

One year of specialized experience (equivalent to the GS-06 grade level in the federal service); experience that equipped the applicant with the particular knowledge, skills and abilities (KSA's) and other characteristics to perform successfully the duties of the position, and that is typically in or related to the work of the position to be filled, in the normal line of progression for the occupation in the organization. Specialized experience is defined as:
* Obtain patient health history, chief complaints, medication history, and identification of health-related conditions that may impact the patient's health status.
* Evaluate patients for emergency medical conditions, order appropriate diagnostic studies and perform a wide range of treatment procedures under the direction of the supervising physician.
* Knowledge of topographic anatomy, e.g., right/left, surface, proximal/distal, superior, inferior, to accurately transmit location information about the body.
* Provide necessary interventions based upon patient symptoms and established protocols/standards of care should be part of the routine capabilities of this position.
* Assists in behavioral/psychiatric situations to include crisis intervention, management of aggressive or suicidal psychiatric patients, and one-to-one observations.
* Knowledge of diagnostic signs, e.g., pulse, respiration, blood pressure, temperature, skin color, pupils of the eyes, state of consciousness, ability to move on command, reaction to pain, etc.
* Practical knowledge of the major body systems, i.e. skeletal, muscular, nervous, respiratory, circulatory, digestive, and genitourinary, to perform patient examination.
* Knowledge of protocol-approved list of medications (their uses, does, side effects and toxicity signs).
* Knowledge sufficient to understand legal considerations of emergency care i.e., actual/implied consent, Good Samaritan laws, etc.Note: The applicant may substitute education for experience if they have successfully completed one full year of graduate level education and it directly relates to the work of this position. Prior military experience as a medic, hospital corpsmen, health service technician, or para rescue specialist. Proficient in both English and Spanish.
